Sven Ulreich's huge error ultimately proved decisive as Real Madrid progressed to the Champions League final thanks to a 2-2 draw in an enthralling semi-final second leg at the Santiago Bernabeu.
Madrid came from behind to win 2-1 last week in the first leg at the Allianz Arena, and sealed their progression with a 4-3 aggregate triumph despite an unconvincing performance on Tuesday.
Bayern took the lead in the third minute, Joshua Kimmich opening the scoring following a mistake by Madrid captain Sergio Ramos.
The hosts were quick to reply but it was Karim Benzema and not Cristiano Ronaldo, making an outfield record 152nd appearance in the competition, who took centre stage.
Benzema's close-range header ensured Madrid's deficit lasted only eight minutes, and 21 seconds into the second half he was presented with a gift when Ulreich misjudged a back-pass from Corentin Tolisso.
There was another twist as James Rodriguez, on a two-year loan spell at Bayern from Madrid, levelled the tie on the night with a goal he did not celebrate but, despite intense pressure from the away team, Zinedine Zidane's men held on to move within a win of a third successive European title, and now await Liverpool or Roma in the Kiev final on May 26.

Cristiano Ronaldo set yet another Champions League record on Tuesday when he made his 152nd career appearance in the competition.
The Portugal star started the second leg of Real Madrid's semi-final against Bayern Munich, with the Blancos carrying a 2-1 lead from the first leg.
With the appearance, Ronaldo broke the record previously held by Barcelona legend Xavi for the most games played by an outfield player in Champions League history.
Former Madrid goalkeeper Iker Casillas has the record for most appearances by any player with 167, meaning Ronaldo is on course to beat that mark in the 2019-20 season.
Ronaldo set another Champions League record just last week, becoming the player with the most wins in competition history.

After helping Real Madrid to the Champions League final, Marcelo has conceded that he was perhaps fortunate to get away with a handball shout during a clash with Bayern Munich.
The German giants felt that they should have been awarded a spot-kick during the closing stages of the first half at Santiago Bernabeu.
With the contest locked at 1-1 on the night, Bayern were pushing hard for a crucial second away goal.
As Joshua Kimmich attempted to fire over a cross, Marcelo closed him down and appeared to touch the ball with his hand while trying to block the delivery.
Bayern’s appeals fell on deaf ears, with the match officials waving away their protests, and Marcelo says it was up to the referee to determine whether his actions had been deliberate.
He spoke to Fox Sports after helping Real to a 2-2 draw and a 4-2 aggregate success and said when asked if the ball had struck his hand: “Yes, it hit my hand.”
When quizzed on whether a penalty should have been awarded, the Brazil international added: “Well, if it hit my hand, I'd say so.
“But you guys know that I don't talk about the referees. Many times, refs have ruled against us, and now, I can't come here and talk about his performance again, because when they rule against us I don’t talk about it, you know. I imagined you'd ask me that..."

Manchester United are willing to listen to offers for defenders Daley Blind and Matteo Darmian this summer, according to reports.
United exercised an option to extend Blind's contract by a further year in February in order to prevent the Netherlands international leaving on a free transfer in the summer.
Blind has featured 16 times for United this season, but has not started a match in the Premier League since August.
The versatile 28-year-old, who joined United in a £13m deal from Ajax in 2014, was understood to be keen to join Roma in January as the Giallorossi searched for a replacement for Emerson Palmieri after the Brazilian left to join Chelsea.
Darmian, meanwhile, has just over a year remaining on his current deal at Old Trafford and previously attracted interest from Roma and Napoli in the January transfer window.

"David has always been open in the communication about that possible move. We discussed that openly and the fact it had failed to happen. I felt he knew exactly what was going on and switched immediately again to his current situation."

Stoke City have announced that Jese Rodriguez’s loan spell at the club has been terminated, with the Paris Saint-Germain forward released on compassionate leave.
The 25-year-old linked up with the Potters in August 2017 after being deemed surplus to requirements in the French capital.
A spell in the Premier League was intended to rejuvenate a player who has seen his career stall since leaving Spanish giants Real Madrid.
Jese was, however, to record just one goal in 13 appearances for Stoke as he struggled to juggle his commitments on and off the field.
He was freed by Stoke to spend time with his hospitalised son in Madrid earlier in the season, but was rarely seen at the club between October and January.
Paul Lambert did bring him back into the fold upon succeeding Mark Hughes at the best365 Stadium, but he took in just four more appearances before disappearing again.
